St. George’s Healthcare Foundation Trust provides a tertiary service and local care to children up to nineteen years of age.
We are looking for an experienced children’s nurse who have the skills and experience to deliver expert nursing care, advice and support in facilitating children and families to manage their long term health conditions proactively within Diabetes speciality .The posts will allow you to become an autonomous practitioner leading and developing the speciality service for the children. You will provide teaching and education to the families and the multidisciplinary teams across primary, secondary and social care sectors. The post holders will deliver nurse care within the hospital wards, outpatient clinics and community setting with telephone advice to enhance patient care and access to advice.
It is expected you will have previous experience and training in the relevant speciality and be willing to continue your education in this specialist area..
This post is supported by identified consultants and multidisciplinary teams to provide holistic care to the children and families. The post works closely and is part of the Paediatric CNS team within the trust. There is commitment to develop new ways of working to increase the levels of care within the community setting to prevent admission and support the MDT. The post holder is expected to be an experienced and skilled children’s nurse who has the ability to have a vision for community care going forward.
