We have an exciting opportunity for a 6 month secondment to support the expansion of the service in to the Emergency Department.
An innovative ‘front door’ service is looking for a dynamic and committed nurse to join the service at St George’s University Hospitals NHS Foundation Trust.
The purpose of the service is to provide early specialist assessment of the older patient in AMU and the Emergency Department and to ensure the most appropriate pathway for their care. As well as nursing staff, the team comprises of consultant geriatrician and therapy staff. This service will operate 7 days a week to meet the needs of patients, to facilitate early supported discharge, admission avoidance or hospital admission as appropriate.
By implementing a ‘front door’ model, a means for early identification and treatment of frail older people can be put in place. Research done by Alio et al (2011) showed that the introduction of a Geriatrician-led in-reach service reduced their length of stay without increasing readmissions.
The nursing team will comprise of a Lead Nurse, Senior Nurses x2 and will be supported by dedicated consultants and therapists.
We are looking for an experienced senior staff nurse with skills in caring for the older person. They should be highly visible, proactive and act as a clinical resource ensuring the sustained delivery of high standards of nursing care and service delivery. You will play a supportive operational role and maintain a proactive approach to managing patient flow through the Service. You will ensure that patients and their families are fully informed and supported by providing clinical and social support. You will have the opportunity to develop in your role as some cover for the Lead Nurse will be expected.

You must have appropriate UK professional registration.
